Outpatient Services
Initial evaluation is $280.00. This 90 minute session includes:
Diagnosis (commonly treated)
Development of a treatment plan
Immediate first treatment
Supply Fee for items used during course of your treatment plan.
Based on your individual treatment plan, the therapist will determine the frequency needed for follow-up appointments. Price for 60-minute follow-up appointments is $176.00. All appointments are scheduled in 30min, 45min, or 60min time slots and are a fixed rate based on treatment time. ( 30 Min. $88; 45min $132)
During your initial evaluation and at each appointment, your therapist discusses with you the most appropriate treatment plan for your personal situation.

Other Programs
Wellness Program - The Wellness Program begins with a 60-minute Wellness Assessment ($180); established patients transitioning from PT do not require the assessment. Follow-up Wellness Visits are $88 for each 30-minute appointment. The frequency of these appointments depends on you. Your therapist will suggest monthly, semi-monthly, or weekly appointments based on your individual goals for the program.
Pregnancy Plan - The initial pregnancy plan visit is $280. During this visit, the therapist will perform an initial assessment, discuss your health goals during pregnancy (and after), and evaluate any risk factors that may be present. The therapist will provide an initial treatment and individualized exercise program. The pregnancy plan continues throughout your pregnancy and for 3 months post-delivery. It includes 60 min discounted visits at $132 per visit.
Yoga Therapy - Yoga Therapy is now available at Penrose Physical Therapy. We are proud to announce that Lisa Phelps, yoga therapist , has joined our team and is offering 2 classes through the clinic, a postpartum class, and a cancer care class, in addition to personal yoga therapy sessions. Find out more about our yoga therapy offerings by clicking the yoga therapy link in the title. 6 week class sessions are $210 ($35 individual class rate). Private sessions are $75 for private hour long sessions.
Additional Information
Cancellation/No show policy: We ask for 24 hours notice if you need to cancel or reschedule. If you are unable to give 24 hours notice or you don’t show for your appointment, you may be subject to a $44 cancel/$88 no-show fee.
Supply Fee: A one-time $35.00 fee will be added to your first visit of the year to help cover the costs of gloves, tape, needles or other supplies used during your treatments. This fee is already incorporated into the price of the Initial Evaluation.
Administrative Fee: There is a fee of $35/month to obtain itemized statements for insurance purposes.
